8 - Pausing and resuming a Patient's Care Plan : Use Case diagram
<b>Pausing a CarePlan</b><br/>The Patient has communicated to the responsible Clinical Monitoring Team that a pause in the active CarePlan is needed. <br/><br/>This can be done by the Patient Solution creating a Communication Resource to the team. <br/><br/>Alternatively, the agreement can be reached using other means of communication like contacting the team by phone.<br/><br/>Using the Clinical Solution, a Practitioner from the team pauses the CarePlan - interacting with the Patients CarePlan resource <br/><br/>The Infrastructure creates an event, signaling the Pause automatically. This Event can be used by Solutions to create communication to the involved parties. Examples include:<br/><br/>The Patient<br/>The Responsible Clinical Monitoring Team<br/>The Care Responsible Organization (Behandlingsansvarlig) <br/>The Administrative Team (that fulfilled the Referral)<br/>The Service Responsible SSL team<br/><br/><b>Resume CarePlan</b><br/>The steps below assume that the Patient has a paused CarePlan assigned to her Episode of Care Resource.<br/><br/>The Patient has communicated to the responsible Clinical Monitoring Team that she wants to resume a paused CarePlan. This can be done by the Patient Solution creating a Communication Resource to the team. Alternatively, the agreement can be reached using other means of communication like contacting the team by phone.<br/><br/>Using the Clinical Solution, a Practitioner from the team pauses the CarePlan - interacting with the Patients CarePlan resource.<br/><br/>The Infrastructure creates an event, signaling the change in the CarePlan status - now becoming active again.<br/><br/>
