The Patient has communicated to the responsible Clinical Monitoring Team that she needs to close the Episode of Care and active CarePlan. <br/><br/>This can be done by the Patient Solution creating a Communication Resource to the team.<br/><br/>Alternatively, the agreement can be reached using other means of communication like contacting the team by phone.<br/><br/>Using the Clinical Solution, a Practioner from the team closes the CarePlan - interacting with the Patients CarePlan resource.<br/><br/>The Infrastructure creates an event, signaling the change to the CarePlan. This can be used by a Clinical Solution to send Communication to parties involved with the Patient's CarePlan. <br/>
