Changes with FHIR R4 Migration

The entire eHealth Implementation Guide has been migrated from FHIR STU3 to FHIR R4. Core migration changes described in https://www.hl7.org/fhir/r4/diff.html are mentioned in the changelog entries below only when significant to eHealth use. For instance, such mentioning could be that another FHIR element, possible introduced with FHIR R4, is now used instead of a FHIR STU3 element.
