When the Patient uses a CarePlan she does so through a Citizen Solution, optionally connected to Devices.<br/><br/>The main processes in this use case are:<br/><ul>
<li>collecting measurements, through Devices or data entry</li><li>answering Questionnaire by submitting Questionnaire Responses</li></ul>
<br/>By using the CarePlan, and associated resources, like the ServiceRequest, the Citizen Solution is able to determine what steps the Patient needs to be guided through, and how often.<br/><br/><br/>
